Complete the information below to apply for the Underrepresented Minority Student Fellowship Application for the Summer 2017 START internship program.

Applications are due alongside Summer 2017 Internship Applications. Applicants must also apply to the internship program separately.

Supplementary Materials

In addition to this form, you must include the following documents in PDF files in this order:

  1. A one-page statement of goals and purpose;  
  2. A letter of recommendation from a faculty member from your home institution, and;
  3. A copy of your FAFSA (or equivalent statement of financial need) for the 2017-2018 academic year.

You must include all of these files to submit this application.  These files must be in (.PDF).

Failure to electronically sign this document is grounds for refusal of admittance into the Underrepresented Minority Student Scholarship Program. If accepted into the program, violating these terms is grounds for removal from the program.
